Following the objective of each level, which is collecting enough hope, the player will guide Anita through three levels: Hearts and Tears Hospital, Pearl Necklace City and Silhouette Street.

Anita must collect every heart of hope in each level in order to access the next one, along with sending menacing rectangle-shape robots known as "Rectbots" to another location with her new cyborg ability.

The beginning of the story features two supporting characters, Vincent Trinians who is Anita's loving uncle and Nurse Nella Indigo who is a nurse with a shocking secret.
